4 post locks, left is broken,right is new. This leads to one of the locks not working and either the cable is constantly lifting the vehicle or the lift gets stuck.

Distracted while lowering the lift
2 Post lock line guide. The sheave had cut into this over a few months. Made it so the locks wouldn't open.
Using the wrong adapter paws can cause the vehicle to be misbalanced and an air hammer or other heavy impacts can cause it to drop
4 post sheave pin, new on top, failed on bottom. Failed pin leads to new sheaves cables and possible steel
Concrete is the most important safety feature of a lift. If the anchors are installed incorrectly (Impact guns, wrong pour, ect..) then the lift will try and twist itself out of the ground. If your anchors can be moved at all you have a major problem!
A lift that hasnt had its cables adjusted, Cables on lifts dont carry weight they equalize a load. Keeping them in adjustment gives years of life to a lift.
Truck racked to far back, puts a ton of wear on the cylinders
Mechanics are human some days are good, some days we rack the vehicle horribly wrong. The above pictures are from a lift that the locks only opened on one side, the tech didn't watch it going down until he heard it.
Lift components wear out over many many years, without someone with knowledge of your lifts looking at them some of these wear points wont be seen until the parts themselves have failed.
